Supporting Lives, Connecting People is the name we use for delivering community-led support to adults in West Sussex.
Community-led support focuses on reaching people at an early stage to help prevent or delay the development of their care and support needs and to enable them to be as independent as possible.
We hold sessions in local communities where people can speak face-to-face with staff from a range of disciplines about their situation and find suitable solutions on the day of the appointment if possible. These sessions are known as Talk Locals and are staffed by social care workers, occupational therapists and Prevention Assessment Team workers.
Talk Locals are held in local venues where assessments (including around bathing, toileting and mobility) can take place, helpful assistive technology equipment can be demonstrated, and information and advice about services and activities in the local community are available. Support for family and friend carers is available and other information and advice, such as about fire safety, equipment and how to get financial advice about paying for long-term care, is on hand.
We support people with lower level social care needs, people who have more complex needs, and family and friend carers.
