- What happens when a transition referral is received?
When a referral is received, the Lifelong Services transition team will:
• Confirmation of diagnosis
We will need some evidence that you have a lifelong disability that started before age 25 (for example: medical evidence of disability, an Education Health, and Care Plan (EHCP), or ‘My Plan’).
• Consider support needs
We will think about whether you may need specialist support, such as from Adult Mental Health Services or the Community Learning Disability Team.
• Look at health needs
We will consider if you may need an NHS Continuing Healthcare (CHC) assessment.
If you have a primary health need and are eligible for CHC funding, your case is normally closed to Adults’ Services.
• Place your referral on the waiting list
If your referral is accepted, it will be placed on a waiting list for a transition care manager to be assigned to the case. Before this happens, someone will contact you to talk about your future plans and goals.
If your referral is not accepted, you will be given a clear explanation, along with advice, information, and signposting to other support.
