Prevention and Early Help
There is an important link between the provision of preventative services and community solutions for people prior to their needs developing into services typically provided by care and support at home services. As part of ensuring that our services can cater for a range of needs and manage increasing demand, we want to work with partners, stakeholders, and the market to ensure that there is a continuum of solutions available with care and support at home being a vital part of this offer that links closely with other solutions.
We have a new Early Help Charter that is a set of overarching principles that West Sussex partners believe will better enable a ‘prevention first approach’ for adults across services in West Sussex; supporting adults to retain their independence for longer and enable efficiencies in care and health services that will help reduce demand for ‘front door’ services. A key part of the Early Help Charter is that customers should be empowered to have more choice and control of their health and care needs through better access to flexible and timely earlier support options.
Access to up-to-date and consistent information on early support services is key to this. WSCC Library Service-managed ‘Council and Local Information Online’ (CLiO) platform provides a digital directory of prevention and early support services at local and county level.
