Planning your support

If you are assessed as having eligible social care needs, a member of staff will talk to you about different options to meet your care needs.   

We will look at sources of support as social care funding cannot pay for things such has NHS-funded care or needs that can be met in other ways, for example, by family and friends. We will also look at whether your needs can be met by linking to resources in the community.

Before the council agrees to fund any service, it will consider all available options to meet your needs.

Reviewing your support

After an agreed period, a member of the Transition team will carry out a review to check if there are any changes to your needs or to your services.

A review is a good opportunity to check that things are going well and that your needs are being met.
