What happens when you leave transition?
When you finish full-time education, your care and support will be reviewed. If everything is going well and you continue to have eligible needs and require a support plan, your case will move from the Lifelong Services Transition Team to your local Lifelong Services Adults Team.
You will still receive support from Lifelong Services, but you will no longer have a transition worker.
If your needs change in the future, or you need a social care review, you (or someone supporting you) can contact your local duty desk.
The Lifelong Services Adults Duty Desk can help with:
- Giving you information and advice
- Taking referrals from carers, service providers, and other organisations
- Dealing with safeguarding concerns (if you do not have an allocated worker)
- Making referrals to the Community Learning Disability (Health) Team
