What happens when a person leaves transition?

When the young person has left full-time education, their package of care has been reviewed, and they are considered settled, their case will then be transferred from the Lifelong Services Transition Team to their local Lifelong Services Adults Team.

The young person remains in Lifelong Services, but they no longer need an allocated transition worker.

If the young person has a change of needs after transition, or requires a social care review, they or their representative, can get in touch with their local duty desk.

The Lifelong Services Adults Duty Desk has many functions, including:

  • Providing information and advice
  • Processing all referrals from carers, providers and external agencies
  • Processing  safeguarding enquiries (if there is no allocated worker)
  • Processing referrals for the Community Learning Disability (Health)Team.
