What does the Lifelong Services Transition Team do?

The team consists of care managers and assistant care managers. Its role is to support young people who are in full-time education and who have a lifelong disability, to seamlessly transition into adult life and any service required as a result of a Care Act 2014 assessment.

By ‘young people’ we refer to people aged 18 - early 20s.

We assess the eligible needs of the young person with their family and carers, planning in partnership how they will achieve their desired outcomes.

This may include providing specialist advice and guidance, signposting, identifying suitable community-led support, support planning and then reviewing the package of support.

We work alongside health and social care professionals, children's services, education, providers and commissioners to achieve person-centred outcomes.

Our aim is to facilitate a positive and well-planned transition for the young person.
