The Prevention Assessment Team (PAT) is a collaboration of partners from the county council, Sussex Community NHS Foundation Trust and Age UK West Sussex, Brighton and Hove. It provides information, advice, and guidance to people aged 18 years and over to enable them to access the support they need to live independently at home and as part of their community for as long as possible. Working with people in their own homes or in community locations, team members carry out assessments with a focus on individuals’ strengths and abilities with the aim of preventing or delaying the need for more intensive services. The right preventative support is identified, including signposting to local services.
In 2023-24, PAT completed 1,213 assessments and received 1636 telephone calls. 1,658 referrals were received (1,636 in 2022-23). Of these referrals:
- 442 were people of working age
- 1203 were for those aged over 65 years
- 13 where age was not disclosed
In 2023-24, the NHS nurse advisors completed 424 health checks (489 in 2022-23). 74% were given advice on cholesterol reduction and 11% were advised to contact their GP for a non-urgent appointment. 70% had advice or were recommended support with weight management. 60 people (14%) were referred to their GP due to high blood pressure and seven required a same day GP appointment.
78 outreach events were attended (44 in 2022-23).
