Residential services - learning disabilities
Directly Provided Services has five residential services across the county for people primarily with learning disabilities.
The service can support people who have some physical and mobility needs; however there are a limited number of rooms that are fully accessible or for those that may need a larger independent area to meet their needs.
The homes promote people’s independence, choice and inclusion, and provide support 24 hours a day. This may include support with:
- personal care
- mobility, fitness and well-being
- managing health issues including medication and attending appointments
- communication and engagement to ensure people are included in all decisions about their care and support
- managing money
- going on holiday.
All services work to shared goals which help to put the person at the centre of their care and support.
- Supporting people to live the lives they want to lead – including work (paid and non-paid), education, and having the opportunity to try new things.
- Support people to access and be involved in their local community.
- Supporting people to maintain friendships and relationships and meet new people.
- Supporting people to receive ‘the right amount of support’
- Supporting people to be involved in everyday decision-making about the support they receive.
- People are at the heart of decision making in the service. This may include participating in staff recruitment, activity development, how money is spent on the service, the environment and equipment, meals and more.
- People are supported to know and understand how to stay safe and healthy.
- People understand how to raise a concern/make a complaint and feel that changes are made.
Hammonds, Tozer House, Ball Tree Croft, and Stanhope Lodge offer short stays, providing a break to the person with a learning disability and to the family member or carer from their caring role. People can choose to stay a few times a year or more often.
